Cognitive Behavioural Therapy Essay Cognitive Behavioural Therapy (CBT) is recommended by the NICE guidelines as an effective treatment for many mental health problems, specifically depression and all of the anxiety disorders. But is it a therapy open to all? When Professor Lord Layard wrote his paper: ââ¬Å"Mental Health: Britainââ¬â¢s Biggest Social Problem? â⬠in 2005, he noted that: ââ¬Å"16% of adults of working age have a mental illnessâ⬠Of these 16%, he stated that, only a quarter were utilising any type of treatment. It was this sort of finding that lead to his recommendation to increase accessibility to proven therapies such as CBT. This was achieved with the roll out of the Improved Access to Psychological Therapy (IAPT) programme. The diagram below shows how the IAPT programme has been divided up into 3 steps: It is this stepped approach that will contribute to overcoming some of the barriers that societies diversity may have with accessing CBT. What might these barriers be? One of the first barriers an individual faces may be around the concern of being stigmatised. It may be too hard for them to think about accessing CBT therapy for fear of what others may think. They may not want to take time off work to attend sessions for fear of it impacting negatively on their career prospects. The stepped care model means they are able to receive the lowest appropriate service tier support. This may mean that an individual is able to access therapy using guided self-help, allowing them to work through the programme in their own time and at their own pace. Online programmes such as ââ¬Å"Fearfighterâ⬠and ââ¬Å"Beating the Bluesâ⬠allow the individual to access therapy where and when they like. Not only do these sort of programmes overcome the fear of stigma but they also overcome the issue of long waiting lists meaning that the individual can access treatment on their terms almost instantaneously. Only if this level of support proves unsuccessful will the individual then be ââ¬Å"stepped upâ⬠to the high intensity support. These internet CBT programmes also overcome geographical difficulties, as long the individual has access to the internet and phone. In my opinion the internet is one of the main ways in which barriers may be removed. Morland et al (2011) conducted a study on the effectiveness of CBT delivered via video conferencing compared to the ââ¬Å"traditional in-personâ⬠. The results indicated that the outcomes were just as effective. A promising move for those that are unable to access traditional CBT as a result of their location or possible mental health issue, for example those suffering agoraphobia. At the core of CBT is the individualââ¬â¢s motivation to learn and change. This in itself can lead to difficulties, especially in children and young people. Often this ââ¬Å"groupâ⬠is being taken to therapy rather than choosing to go. Hudson (2005) found that large numbers of youngsters were not responding to the CBT treatment that they were receiving. As far as children and young people are concerned there seems to have been limited research onto the success of CBT treatment. The first controlled trials did not take place until the early 1990s and the majority of research has focused on the impact of CBT on adults. Even so the NICE guidelines on Depression in Children and Young People recommended the use of CBT, firstly, rather than pharmacological interventions. So how has CBT been adapted to incorporate children and young people, whose very cognition and emotional stages differs from that of adults? There have been some creative adaptations to the CBT model such as ââ¬Å"Think Good Feel goodâ⬠. Here the concept of CBT is broken down into easy to manage bites. There is also a lot of use of cartoons and speech bubbles, allowing the young person to access their thoughts and feelings in a safe way. The need to use more non-verbal techniques to help the young person to engage in the process such as, storytelling, drawing and games is also highlighted in this book. Oââ¬â¢Reilly et al (2009) have also developed an interesting approach to using CBT with children and young people. They have created a game called ââ¬Å"gNAtenboroughs Islandâ⬠which is played by the youngster alongside a therapist. Each session introduces another core CBT concept to the young person, such as the connection between thoughts, feelings and behaviours. These are presented in a non-threatening way and also a way that may be more engaging to this age group. Cerangolu (2010) found that video games could enhance the therapeutic experience. They could help facilitate the therapeutic relationship as young people may be more willing to relate to a therapist that is ready to engage and understand their normal way of playing. Also sitting side by side rather than at the conventional ââ¬Å"ten to twoâ⬠setting also may make the young person relax. How the game is played can also reveal the young person cognitive style. As technology develops so do the opportunities to engage young people in therapy. Apps for smartphones are constantly being developed that allow the young person to access their homework in a more user friendly way, to record their thoughts and emotions instantaneously. To allow them to engage in therapy in the same way that they engage in their life. Homework may be a part of CBT therapy that causes a young person to disengage, especially as the very word may cause them to have negative connotations. Gaynor et al (2006) found that compliance toward completing homework dropped the further into therapy a young person was. It is important that the therapist and client work together to agree the homework and that the young person is encouraged to set their own, as this may encourage a ââ¬Å"buy in ââ¬Å"to its importance. It may be that the therapist stays away from using the language of school and calls it ââ¬Å"practise workâ⬠or ââ¬Å"work for selfâ⬠. Initially I saw CBT treatment as regimented and unmoving, but the more I have researched the barriers that different people may encompass when thinking of embarking on treatment the more I realise that CBT is indeed structured but there is an innate flexibility about how it is delivered. It is this flexibility that allows people from different backgrounds to engage in a CBT approach that sees them as an individual rather than specific disorder.

Materials for Constructing a Multi-Storey Bulding When dealing with a multi-storey building it is important to get the right materials as these materials will hold the structure together, the multi-storey building itself is very large and will be constantly in use as it is a hotel so the building will be subjected to different loads and situations. Loadings on beams, columns, frames, pads and floors can cause many structural differences the behave and performance of the material may change due to stress and strain on them, the materials I am going to look at are steel and concrete, these materials are used throughout the construction industry and loads on these materials are important as if the material is not up to the job and does not fulfil the need the beam, column, frame, pad or floor could collapse and cause many injuries and be a health and safety danger so it is important to use the materials in the right way. both materials steel and concrete have the advantages and disadvantages when used, when dealing with a multi-storey building it is important that the material can withstand the pressure it is put under so it would not collapse or break, if this does occur the knock on effect would result in the building to collapse and cause large injurys to civilians within and around the building, to stop this the materials which are used and put under a subject of tests such as being compressed and stretched rapidly any faults such as the material does not meet to the UK standard would be out lined and amended or discharged from use on site, this is also done for health and safety reasons. First looking at concrete it is used often through put construction it is used due to its durability and workability, to get to a workable mix the concrete needs to be cured and portioned first, it is important that the concrete meets the needs this could be the concrete is used to construct a concrete beam and this beam is put under a lot of loads; therefore the beam would need to be able to receive the load and distributed. Every material has Strength this is the ability of the material to resist the forces set up within them by the applied load, when a load is applied movement is made so therefore Stability is needed stability is the ability of the structure to resist overall movement, any load (force) tends to move to a downward direction for the material to keep to its state it should maintain a equilibrium this is done by the material resisting the equal and opposite force, any load that is applied is called a active force, this force can also be distributed this is when the load is applied over full length or area of a structural member this would be used in a pad foundation, also other loads may be called can be concentrated or point where the load is put to a single point or over a very restricted area this is used with conjunction of columns and beams. When the load is put to a material in a form of a beam, column, frame, pad or floor the structural material may be affected in the following ways: Stretch called the tensile force, tend to stretch the material Compress called the compressive force , which tends to squeeze a material Shear force when the force causes one part to slide past the other force which tends to make surfaces of a material slide. Torsional force causes the member to twist If the load is not distributed this can result in the beam could form cracks and collapse due to the load it is receiving this works in vise versa if the beam is being crushed for example the building could encounter an earth quake and the beam is being crushed the beam needs to withstand this to help keep the structure of the building; so therefore tests are made up such as a crushing test this is at the stage where the concrete is being made/ produced (mixing/curing), the concrete is mixed and cured (curing can take weeks) depending on the ratio used in the mix, the crushing test is the mix is set into cubes and at different curing stages, each cube is put under a load at which could be put on the multi-storey structure, once the results of each cube are collected the results are analysed and then a decision is made in which grade of concrete to work with for the build, the formula for the test is: These tests are made to find of the: Shear forces Bending moment Deflected shape Reactions at supports As you can see from the example I have given as time goes on the crunching load has increased, this shows that the concrete cube has become more diverse in strength by every week, from these crushing loads the different concrete grade can be used at different points in in construction. Stress is derived by dividing the load applied by the cross-sectional area of the member Stress (f) = Load/ Area = W/A= N/mm2 Strain is measured by dividing the change in length by the original length Strain = change in length Original length The purpose of beams, columns, frames, pads and floors are to: Resist loads imposed upon it. Beams can be in many forms such as: Simplyà supported à where the beam is resting on two supports. Fixed where the ends firmly built in and do not move Cantilever à where one is à firmly built in at one end and the other end is free Continuous à where the beam is resting on three or more supports. Aà Columnà is a vertical structural member used in the construction industry to transmità imposed loadsà to other members or to the foundation. Compression, Tension and Shearà When the force on a structural member stretches called the tensile force tends to stretch the material. When a force squashes or compresses the member, this is called compressive force , which tends to squeeze the material When the force causes one part to slide past the other force which tends to make surfaces of a material slide. This is called a shear force Bending, Buckling and Deflection this is most common in columns, columns are mostly made of concrete or steel due to their strength properties, timber may also be used but if a large load it put on the Colum buckling could occur (bend sideways).à A beam could be affected by defection if a large load is put on it, the beam could be made of either steel or timber.à Aà bending momentà is occurs when the material resists a force this occurs with beams. concrete is a mixture of cement, fine aggregate, course aggregate and water when these are mixed at a good ratio to each other when set it will form a hard solid mass or artificial stone, there is a rule which is used in construction the greater the proportion of cement, the stronger strength of the aggregate , but the water content in the cement is important this process happens in curing the cement over a couple of weeks, as there is more water the cement may not be able to be at its full strength, due to the strength, workability and density of concrete it can be easily made and humans have the ability to create concrete with very specific properties, therefore this material is used allot in design and construction where the manufacture produces a mix when set concrete produced will have desired properties needed. The multi-storey building is made of concrete would consist of: Slabs Columns Shear walls Foundations Reinforced concrete suspended floors Ribbed floors Hollow pot floors Beams Precast concrete floors Concrete is easy to maintain, the first and up most important thing to do is to allow proper curing of the mix as this will allow maximum design characteristics of the concrete mix to be achieved, the concrete when set can be covered in a water resistance paint this will protect the set concrete from water as water is concretes worst enemy; if water sits on the concrete this can stain the surface, allow cracks to form and mold spores to develop making the concrete slippery, this could cause major defects in the construction itself,à the most important thing is to look at temperature when dealing with concrete as concrete can expand and contract, which can force cracking and leave the concrete exposed to increased water intrusion so if crakes do occur these should be filled in and dealt with straight away. Concrete helps to minimise the amount of energy that a building consumes and ensure that the building maintains a level of thermal comfort that is appropriate for its occupants. C oncrete helps buildings to achieve this, as a heavyweight material, concrete acts as a store (or buffer) during the heating season by utilising free heat gains, concrete can It can only be renewed if it is crushed down and grinned into sand. Steel is made up of alloy of iron and carbon, steel can be very useful to the industry of construction, it is used from protective shoes to large beams to small nails, the material is so versatile due to its melting and strengths properties but yet it is lightweight for easy handling this means economical shipping. Precise measurements can be taken meaning very precise cuts of steel can be made so less wastage is made, steel acts as Great protection against the worst weather conditions, Installation is fast and simple, Steel is also recyclable so it is environmentally friendly the steel can be easily malted down and re used for another purpose, to maintain steel it would need to be cleaned regularly and checked for any defeats, when using steel in construction it is important to take in account energy efficiency; if a whole home was built of steel it would be less energy efficient than a wooden house, insulation would be needed to keep in heat this is because metal transfers heat to a colder area more quickly than wood sometimes up to 400 times more rapidly steel studs can create a thermal bridge to the outside of the house, to stop this when the house is being constructed the steel can be warped in insulating board this would help to keep in heat and reduce the thermal bridge, but also one adv antage of steel is the fact that steel can be recycled with ease it can be melted down with not a lot of hassle and the steel can be used to make other things. The sustainability of steel is very good as this material allows, rapid construction on site (therefore reducing site activity) and disruption to neighbors, steel can be precut and delivered in so a system called just-in-time can be used this will reduce site congestion, steel structures are inherently adaptable and extendable, and also steel is multi-cyclable, being 100% recyclable without down cycling. Steel is located in the ground so it would need to be dug out to be used as it is made of carbon and iron, When it is all finished there will be no more. That is why recycling steel is so important. Steel is widely used in construction, due to its properties it is used for mostly columns frames , these would be made of entirely of steel; this is good as steel is very strong but when a load is but on steel from both ends such as a column the column could compress and buckling effect could happen, also if the load is great enough column could break (snap) .Ã

On-Star: A Necessary Car Safety Feature Finally, after waiting for three years, my car broke down. I bought a '76 Geo Prizm thinking that it wouldn't be bad to have during High School. A car would be a perfect gift for graduation. Well, because I was going to college, I needed something that would keep me safe and last me for more than three years. In addition, I locked my keys in my car all of the time, and I needed a car with a code on the outside of it so I can receive my keys without calling a cop. I decided to ask my father what type of vehicle I should get. He told me that the most popular type of cars are the ones that are sponsored by On-Star. According to him, On-Star helps in many different ways. Therefore, knowing this, we started our venture to find the perfect car. We went to a couple of car dealerships in Grand Junction, but none of the cars that were offered satisfied me. So, my dad and I planned a trip to Denver to widen my choices. When we first drove into Denver, there were so many car dealerships to choose from. We just started going to them one by one. Finally, after a good four hours, we found the car. This car was the perfect one for me. It was a silver, 2003 Pontiac Grand Prix. My dad wouldn't let me get anything that didn't support On-Star, so that helped lower my options. Both of us were very pleased with my decision. I insisted on having him drive back home, without me behind. I wanted to venture off on my own. I wanted to go visit my friends in Lakewood. I told him I would be back by noon the next day. At first, he was hesitant, but he knew I had On-Star to keep me safe. My dad and I hugged, and said our "See ya later's." We drove in opposite directions, knowing everything was going to be perfectly fine. As I started driving to Emily and Alex's apartment, I started messing with the On-Star button. To my surprise, I heard a voice from the speakers on the roof of the car. She asked if everything was alright. To my embarrassment, I made up a story and told her that I needed directions. Right away, she helped my needs, and told me how to get to their apartment.

These forces were the apparitions, the old witches, and Lady Macbeth. The apparitions influenced Macbeth by making him think he was invincible. The witches influenced Macbeth by telling him that he was going to become king. Lady Macbeth also influenced him by emasculating him. Although these factors influenced Macbeth, it was still his choice to act upon this decision. The apparitions influenced Macbeth by making him think he was invincible. The first apparition said, ââ¬Å"Macbeth! Macbeth! Macbeth! Beware Macduff! Beware Thane of Fife, dismiss me; enough.â⬠(act 4, scene 1, lines 71-73) This meant that Macbeth, had to watch out for Macduff, the Thane of Fife . The second apparition said, ââ¬Å"Be bloody, bold, and resolute, Laugh to scorn the power of man, for none born of woman shall harm Macbeth.â⬠( act 4, scene 1, lines 78-81) This meant that no man that was born out of the female birth canal could harm Macbeth. This statement contradicts the one statement made by the first apparition. Macbeth thought that since Macduff was human that he had to have been born through the birth canal. Macbeth did not know that Macduff was born by c-section, he was untimely ripped. The third apparition said, ââ¬Å"Be lion-mettled, proud and take no care of who chafes, who frets, or where conspires are: Macbeth shall never vanquished be until Great Birnam Wood to high Dunsinane Hill shall come against him.â⬠(Act 4, Scene 1, Lines 89-94) This means that Macbeth should be brave and proud. It also means that Macbeth will not be killed until the forest marches up to his castle. Macbeth thinks though that that was absurd. He never imagined the forest walking. The forest did walk though. It walked when Macduff and his associates camouflaged themselves with it, and attacked Macbethââ¬â¢s castle. The apparitions made him feel like he could do anything. Then he decided to take out Duncan. à à à à à The witches also influenced Macbeth in killing King Duncan. They told Macbeth that he was going to become king. Cultural Counseling and Latino Population Essay
A counselor must understand how oneââ¬â¢s own cultural differences and worldviews impact the counseling relationship. This paper will examine how I am culturally different from my client and how I would demonstrate cultural competence. Furthermore, I will describe how I can improve my cultural competence with the client. Clients Background Information My client, Alberto, is a 47-year-old Mexican American who has been married to his wife, Angela, for 27 years and has four children. Alberto lost his job and their savings is limited. Angela desires to work to help provide for the family. Alberto refuses to allow his wife to work. Both seek counseling, primarily because their priest has recommended them to do so. Alberto displays resistance to counseling because he believes it is a waste of time. Cultural Differences Counselors need to be mindful of how their own worldviews, biases, and prejudices impact the counseling relationship. By examining the Latino culture and comparing it to my own enables me to be aware of the differences and prevent me from judging and influencing my cultural worldviews on my clients. I would be respectful to their worldviews and incorporate their values in the counseling process. Latino Cultural Worldviews. Albertoââ¬â¢s characteristics heavily reflect traditional Latino family values. The Latino family structure culture is mainly hierarchal, with the father the head and main provider of the household with machismo characteristics such as being dominant and strong (Sue & Sue, 2013). The mother takes on more of the caregiver role, or marianismo characteristics, and focuses on raising the children (Sue & Sue, 2013). The traditional Latino husband/father expects respect and obedience from all the family members. Counselorââ¬â¢s Cultural Worldviews. I grew up in a multiracial family of four.à Both my parents worked very hard, with my mother being the ââ¬Ëbread winnerââ¬â¢ of the family. This is an unusual characteristic in a military household because normally the mothers stay home since constant moving around the country complicates a womanââ¬â¢s career development. It is also unusual in both the Italian culture and Korean culture for the woman to be the main provider in the household. Both my parents acculturated in their own ways before marrying, and swayed from traditional values of their bloodlines. Everyone in the family had a contributing role; whether it was financially or helping around the home, everyone shared responsibility. My parents encouraged my sister and I to work hard and strive for our dreams- whatever it was they supported us. There were no given roles in our family. Rather, our family culture carried a democracy atmosphere where we all worked and made decisions together. Cultural Differences. The main cultural difference is our family systems and the gender role expectations of father and mother. In the traditional Latino culture, the man dictates the household. Whereas in my family, roles are harder to define since my father and mother shared financial and household responsibility. Demonstrating Cultural Competence Latinos often times disagree with gender roles. This is the case for Alberto and Angela. Albertoââ¬â¢s unemployment is causing strain for the family in terms of financial burden and emotional distress. Challenging the father, or machismo, opposed their traditional values. Avila and Avila (1995) and Constantine, Gloria and Baron (2006), noted that traditional Latino men struggle with gender role conflict when experiencing: (1) lack of confidence in areas of authority, (2) feelings of isolation and depression because of the need to be strong, and (3) conflicts over the need to be consistent in his role (as cited in Sue & Sue, 2013, p.413). Albertoââ¬â¢s openness to seek counseling is limited. In the Latino culture, counseling can be viewed as weak (Sue & Sue, 2013). Seeing as Angela wants to work to help make money for the family, conflicting views about roles and expectations has surfaced between the couple. Angelaââ¬â¢s conflict revolves around living up to the traditional L atina expectations; meaning that she is primarily a homemaker. Also, Angelaââ¬â¢s opinions and voice is discouraged by her Alberto in the counseling session. This demonstrates how many traditional Latinas are inferior in the marriage (Sue & Sue, 2013). Andres-Hyman, Ortiz, Anez,à Paris, and Davidson (2006), advised that counselor must look at the clientââ¬â¢s degree of devotion to traditional gender customs (as cited in Sue & Sue, 2013, pp.413-414). As Alberto and Angelaââ¬â¢s counselor I would explore their roles within their family and delve into how these roles may be altering and impacting the family. Also, I would address Albertoââ¬â¢s anxiety levels. Furthermore, I would help Angela consider the change involved with assuming a new role in the family. By addressing these issues in the coupleââ¬â¢s cultural framework, I would hope to accomplish a understanding and possible compromise. Improving Cultural Competence with the Latino Population Coming from an equal relationship belief, I am challenged not to favor Angelaââ¬â¢s desire to work and contribute financially. My perspective clashes with the traditional Latino gender role expectations. I must remind myself that my beliefs are not universal and not best for everyone. One way to improve my cultural competence is to respect the traditional Latino worldview. Since Alberto is a traditional man, I would respect his values of authority and interview him briefly to demonstrate respect (Sue & Sue, 2013). Also, I understand that in a less acculturated family that I need be more formal and less casual. Reflection Cultural competent practices are essential in the counseling profession. Developing an understanding and respect for people with differing worldviews and cultures promotes the counseling relationship. Practicing within the clientââ¬â¢s cultural framework is necessary. Understanding oneââ¬â¢s own culture and being mindful how it differs from others is primary. Identifying the cultural differences allows the counselor to be mindful and delineate from imposing oneââ¬â¢s own beliefs onto the client. References Sue, D. W., & Sue, D. (2013).
